As our community ages and experiences multiple health issues, it becomes increasingly important to provide care in line with a person’s wishes.

Aged care workers and general practitioners can provide care that is patient-centred and is delivered in line with a person’s goals, beliefs and preferences – particularly as people age and become more likely to have multiple chronic conditions.
